Are agencies measuring the right things — and hitting them?

Measures are classified deterministically as input / output / outcome / efficiency. An agency whose measures are mostly inputs and outputs is tracking activity rather than results for the public. Target hit-rate uses the agency's own published targets. We report only what the data supports.
